This notice covers this local website for Any Lab Test Now Everett (1205 SE Everett Mall Way, Suite D, Everett, WA 98208) and the visit requests you send through it. The LA Fitness Everett Wellness Pass has its own privacy policy.
What we collect when you request a visit
Your name and mobile phone number; your email address, if you give one; the testing categories you choose and any specific tests you tick, or that you are not sure yet; the name of a test you could not find, if you tell us one; for drug and alcohol testing, whether an employer or testing program arranged it; your preferred day and one-hour window; and the optional note you write.
How you found us
We also record how you arrived, so we know which listings and links bring people to us: the tracking tag in the link you followed (for example from a map or directory listing), any campaign labels in that link, the address of the site you came from (without anything after a “?”), the first page you opened here, and when.
How we measure these pages
We count how these pages are used — for example how many visits open the request form, how far they get through its three steps, and whether someone taps to call or for directions — using an anonymous, random visit id that lives in the cookie described below. Those counts are kept in our own system with the tracking tags described above. They never include your name, contact details, the tests or categories you choose, or anything you type. When you send a request, the record of that visit is linked to your request, so we can tell which listings lead to visits.
What we don’t ask for
We don’t ask for your date of birth, insurance details, Social Security number, symptoms, diagnoses or medications, and we ask you not to put medical information in the note. This website does not receive or store lab results.
How we use it
To arrange and confirm the visit you asked for. When you send a request, our staff are notified by email with your details, and we may contact you by phone, text message or email about that request.
Text messages
If we text you about your request, the message comes from Any Lab Test Now Everett through our text messaging provider. Reply STOP to stop texts from us. Requesting a visit does not sign you up for marketing texts or emails, and does not add you to a mailing list.
Cookies and other services
These pages set one cookie, which keeps the request form secure, remembers how you arrived and holds the anonymous visit id, until you close your browser. We don’t use advertising or third-party analytics trackers on these pages. The pages load fonts from Google Fonts, so Google receives your browser’s network address when they load.
